Cook Willow Health & Rehab Center is privately owned & operated since 1950, is a skilled nursing facility, specializing in Short term rehab, long term and respite care. Cook willow provides 24 hour nursing care in a safe, comfortable and home like setting, located in a peaceful rural area. We offer privacy and convenience for residents and their families, and provide care for residents who no longer need the services of an acute care hospital, but who are still in the need of intensive services before returning home. For those residents who are unable to return home we offer a smooth and easy transition from home to long-term care. Our care and treatment is "resident centered" delivered by dedicated professionals and thoughtfully planned according to resident needs and lifestyles.
